MR Spectroscopy of the Brain

Sometimes a spot on a brain MRI could be several different things, such as a tumor, an infection or changes from past radiation treatment. MR spectroscopy adds a look at the chemistry inside that spot, which can help tell them apart without surgery.

MR spectroscopy, or MRS, is an extra sequence added to a brain MRI. Instead of making a picture, it measures the levels of certain natural chemicals in a small area of the brain, such as:

• NAA, a marker of healthy nerve cells
• Choline, which rises when cells are dividing quickly, as in many tumors
• Creatine, a marker of energy use
• Lactate and lipids, which can point to areas of dead tissue, infection or very aggressive tumors

The pattern of these chemicals is shown as a graph with peaks, which the radiologist interprets.

MR spectroscopy can help:

• Tell a brain tumor such as a glioma apart from a brain abscess or other non-tumor problems
• Estimate how aggressive a tumor may be
• Tell tumor regrowth apart from radiation changes after treatment
• Choose the best spot for a brain biopsy

Dr. Grunch may request MRS when a brain MRI shows a spot that needs more explanation before deciding on a biopsy, surgery or watchful waiting. The same technology is behind Nociscan, which uses spectroscopy to study the discs of the lower back.

• Prepare the same way as for a regular MRI, including telling the imaging center about any implants and leaving metal at home.
• MRS is often done with a contrast MRI, so you may need to follow those steps too.
• No special diet or medicine changes are usually needed.

MRS feels exactly like a regular MRI. It adds about 10 to 15 minutes to the scan while you lie still. You will hear the same knocking and buzzing sounds.

You can go back to normal activities right away. The radiologist includes the spectroscopy results in the MRI report, and Dr. Grunch or one of her physician assistants will go over them with you along with the pictures.

MR spectroscopy is as safe as a regular MRI. It uses no radiation. The only safety concerns are the same as any MRI: metal and certain implants, plus contrast if it is used.
